• Single - Single applicant between the ages of 18 and 64 - $125.00
   

• Family - Includes husband and wife - $140.00
   (Children under 18 years of age require an additional insurance fee of $15.00 per child)

• Senior - 65 years and over - $65.00
   

• Junior - Under 18 years old - $65.00
   

• Initiation - Additional one time fee for all new applicants - $25.00

          (Initiation fee is waived for Junior applicants)
